Please read the full Rules and Regulations of the Sweepstakes. Keep in mind that you must accept the Rules in order to enter the LEGENDS OF JAZZ Sweepstakes.

LEGENDS OF JAZZ SWEEPSTAKES OFFICIAL RULES AND REGULATIONS

SWEEPSTAKES DESCRIPTION:  The LEGENDS OF JAZZ Sweepstakes ("Sweepstakes") is sponsored by LRSmedia, LLC, Suite 610, 350 West Hubbard, Chicago, Illinois 60610 ("Sponsor"). The Sweepstakes begins at 12:00 AM Central Time ("CT") on January 10, 2007, and ends at 11:59 PM CT on February 1, 2007 ("Promotion Period"). One (1) Grand Prize winner and five (5) Second Prize winners will be selected in a random drawing from among all eligible entries received during the Promotion Period.
ELIGIBILITY: The Sweepstakes is open only to legal residents of the 50 United States and District of Columbia ("U.S.") who are 18 years of age or older as of date of entry ("Entrant"). Employees, directors and officers and their immediate families (spouses and siblings, parents and children and their spouses) and household members of LRSmedia, LLC ("Sponsor") are not eligible. Sweepstakes is void in Puerto Rico, and other US Commonwealths, Territories and jurisdictions (including overseas military installations) and where prohibited.
AGREEMENT TO THE OFFICIAL RULES: By participating in the Sweepstakes, each Entrant (or if the Entrant is not of the age of majority in his/her state of residence [an "eligible minor"], their parent or legal guardian) fully and unconditionally agrees to and accepts these Official Rules and the decisions of the Sponsor, which are final and binding in all matters related to the Sweepstakes. Winning a prize is contingent upon fulfilling all requirements set forth herein.
HOW TO ENTER: a) visit www.legendsofjazz.net/IAJE, fill in the online entry form and submit it as directed; or b) hand print your name, complete address (P.O. Boxes are not accepted), age and phone number on a postcard and mail it to LEGENDS OF JAZZ Sweepstakes, Suite 610, 350 West Hubbard, Chicago, Illinois 60610.  There is a limit of one (1) entry per person by any individual method of entry during the Promotion Period. Any attempt by any participant to obtain more than the stated number of entries by using multiple/different email addresses, identities, registrations and logins, or any other methods will void that participant's entries and that participant may be disqualified. Limit one entry per envelope. Mechanically reproduced entries are not permitted.  All entries become the exclusive property of Sponsor and will not be acknowledged or returned. Proof of sending will not be deemed to be proof of receipt by Sponsor.  In the event more than one (1) entry is received from any one person or email address, the first entry processed will be considered the only valid entry received for the Promotion Period. In the event of a dispute, entries will be deemed made by the authorized account holder of the email address submitted at the time of entry. The authorized account holder is the natural person who is assigned to the email address by an internet access provider, online service provider, or other organization that is responsible for assigning email addresses or the domain associated with the submitted email address. The potential winner may be required to show proof of being the authorized account holder.
WINNER SELECTION AND VERIFICATION: On or about the 10th day following the Promotion Period, one Grand Prize winner will be selected in a random drawing from among all entries received during the Promotion Period.   Following selection of the Grand Prize winner, five (5) Second Prizes will be selected in a random drawing from among all entries received during the Promotion Period other than the winner of the Grand Prize.  Random drawings will be conducted by Sponsor whose decisions on all matters relating to this Sweepstakes shall be final and binding.  Potential winner will be contacted by the Sponsor by telephone and email and will be required to complete and return an Affidavit of Eligibility and Liability Release and except where prohibited by law, a Publicity Release Form ("Affidavit/Release") within seven days of receipt. If the potential winner is an eligible minor, at Sponsor’s discretion Affidavit/Release must be confirmed and signed by his/her parent or legal guardian. Failure to timely return any required executed documents may cause prize to be forfeited and awarded to an alternate as solely determined by Sponsor. If a prize is refused, a prize notification is returned or deemed undeliverable, or potential winner is disqualified for any reason, an alternate prize winner will be selected. Except where prohibited by law, acceptance of a prize constitutes permission for Sponsor to use winner’s name, hometown, likeness, statements and other personally identifiable information for promotional, advertising and marketing purposes in any media throughout the world without additional prize, incentive, consideration, consent or review; and upon request, winner will provide written consent to such use. Odds of winning depend on the number of eligible entries received.
PRIZES / APPROXIMATE RETAIL VALUE (ARV): One (1) Grand Prize and five (5) Second Prizes will be awarded during the Sweepstakes.  The Grand Prize consists of one (1) copy of the LEGENDS OF JAZZ DVD and CD Box Set autographed by Ramsey Lewis (ARV $200.00).  Each Second Prize consists of one (1) copy of the LEGENDS OF JAZZ DVD and CD Box Set (ARV $20.00).
PRIZE RESTRICTIONS:   Prize may not be transferred or assigned. No substitution of prize may be made, except by the Sponsor, who reserves the right to substitute a prize in whole or in part with another prize of comparable or greater value. Winner will be responsible for all taxes (not limited to federal, state, local and/or income) on prize and will be required to provide his/her Social Security number for tax reporting purposes. Sponsor will file where required an IRS Form 1099 with the Internal Revenue Service for the fair market value of a prize.
NO LIABILITY: By participating, Entrants agree to release, discharge, indemnify and hold harmless the Sponsor and its officers, directors, employees, representatives and agents from and against any claims made by winners, Entrants, or any other third parties, related in any way to the operation of this Sweepstakes as well as any other claims, damages or liability due to any injuries, damages or losses to any person (including death) or property of any kind resulting in whole or in part, directly or indirectly, from acceptance, possession, misuse or use of any prize or participation in any Sweepstakes-related activity or participation in this Sweepstakes.
GENERAL CONDITIONS: This Sweepstakes is subject to all applicable federal, state and local laws. By participating, Entrants agree to be bound by these Official Rules and the decisions of the Sponsor and waive any right to claim ambiguity in the Sweepstakes or these Official Rules. Sponsor is not responsible for lost, late, misdirected, undeliverable or incomplete entries, whether due to system errors or failures, faulty transmissions or other telecommunications malfunctions, entries not received resulting from any hardware or software failures of any kind, lost or unavailable network connections, failed, incomplete or garbled computer or telephone transmissions, typographical or system errors and failures, faulty transmissions, technical malfunctions, or otherwise. Sponsor may prohibit an Entrant from participating in the Sweepstakes or winning a prize if, in its sole discretion, it determines that said Entrant is attempting to undermine the legitimate operation of the Sweepstakes by cheating, hacking, deception, or other unfair playing practices (including the use of automated quick entry programs) or intending to annoy, abuse, threaten or harass any other Entrants, or Sponsor.  CAUTION: ANY ATTEMPT BY AN ENTRANT TO DELIBERATELY DAMAGE THE WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES MAY BE IN VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK REMEDIES AND DAMAGES (INCLUDING ATTORNEY'S FEES) FROM ANY SUCH ENTRANT TO THE FULLEST EXTENT OF THE LAW, INCLUDING CRIMINAL PROSECUTION. Failure to enforce any term of these Official Rules shall not constitute a waiver of that provision. If for any reason the Sweepstakes is not capable of being executed as planned, or infection by computer virus, bugs, tampering, unauthorized intervention, fraud, action of Entrants, technical failures or any other causes which in the opinion of Sponsor, corrupt or affect the administration, security, fairness, integrity, or proper conduct of this Sweepstakes, Sponsor reserves the right at its sole discretion to disqualify any suspect entry or Entrant and to cancel, terminate, modify or suspend the Sweepstakes. In the event of any cancellation, termination or suspension, notice thereof will be posted at the Legends Of Jazz website at www.legendsofjazz.net and the determination of the Grand Prize or Second Prize winner will be made, from among all eligible, non-suspect entries received as of the date of the termination, cancellation or suspension, as Administrator determines in its sole discretion. Entries not complying with all rules are subject to disqualification.
ENTRANTS PERSONAL INFORMATION: Information collected from Entrants is subject to Sponsor’s Privacy Policy (http://www.legendsofjazz.net/footerlinks?id=0002).
WINNER / RULES: For the name of the Sweepstakes winner and/or a copy of these Official Rules, mail a stamped, self-addressed envelope to: LEGENDS OF JAZZ Sweepstakes Request, Suite 610, 350 West Hubbard Street, Chicago, Illinois 60610. Specify "winner list" or "rules" on your request. Requests must be received by 6/30/07.
